A fully completed WC Acord 130, edition 8/2000, signed by either the agent or insured must be submitted. Our underwriting staff will qualify your submission for program eligibility and pricing to determine final underwriting acceptability for a coverage quotation proposal. All information, including the FEIN number, officer/partner information, number of employees by class, prior carrier premium and loss data along with a meaningful description of operations must be included. Finally, we require the most current experience modification worksheet with currently valued loss runs along with explanatory remarks for any yes answers in the general information section.
